Enclosed you will find a transfer from which you have to fill out in block capitals, please make sure that it is readable
if you want us to transfer the money into a bank account which is not yours please make sure that you fill in the name and address of the allottee
you have to sign this form yourself and we will check the signature with the one on the authorisation form
if somebody else will sign we will not pay out the money this to avoid any problems in the future. you can send the transfer form back by post, fax (+31 10 423 39 33) or by 3-mail to klein_debbie@itf.org.uk.as soon as we have received the transfer form back into our office and everything is correct we will transfer money into your given account
